It might be helpful to come up with some systematic way of changing the case of letters perhaps based on date, because if you accidentally re-use a pattern too soon, you will get the stale cached copy.

What I don’t get is why they can’t or won’t fix it! If you can’t post the current image and it is not damaging in any way then something isn’t working right.
For reasons that make sense to them, whenever you post an image inline in comments here at wordpress, they first check if anyone had referred to that image before. If not they then take a snapshot of it, save it on their site, and refer to that. So subsequent references to the same URL will wind up going to the original snapshot they took.
The URL comparison is case sensitive. But the first portion of any URL which winds up being mapped through DNS is NOT case sensitive yet. This is why the trick of uppercasing some characters of the initial part of the URL works to make wordpress think that particular URL had not been referred to previously.
In some cases this could actually be a good thing, if someone tries to delete an image that you referred to, and you want to be able to see the original. By their making a copy, deletion of the original will not affect being able to see it here.
While it may be an annoyance, it is easy to work around, and in some cases it can work to our benefit.
